OUR SERVICES

Home health care services must be ordered and certified by your primary care physician. Our interdisciplinary team of nurses, therapists and aides will work closely with your physician throughout the duration of your care. ​

WE PROVIDE THE FOLLOWING SERVICES IN THE PATIENT'S HOME:
  • Skilled Nursing Care by a Registered Nurse and/or Licensed Practical Nurse
  • Home Health Aide Service
  • Physical Therapy*
  • Occupational Therapy*
  • Speech-Language Pathology*
  • Medical Social Service*

OTHER SERVICES:
We also assist patients and connect them with available community resources should they need assistance in getting the following services:
  • Homemakers, Meals-on-Wheels and Transportation Assistance 
  • Durable Medical Equipment and Supplies
  • Home Diagnostic and Laboratory Services
  • Dietitian and Podiatry Services

OUR SPECIALTIES INCLUDE:
Geriatric Care, Wound Care,  Palliative Care (not hospice), Cardiac Care and Post Stroke Disorders, Alzheimer's/Dementia Care, Diabetes Care, Home G-Tube Care and Education, Pulmonary/ COPD, Osteoarthritis Care, Orthopedic Physical Therapy and Progressive Neurological Conditions.​
*Provided by contracted therapists who cover the patient's area of residence.
