Information for Healthcare Professionals

When referring a patient, we will require a home health order, progress notes and face-to-face encounter attestation from the patient's primary care doctor or nonphysician practitioner who must be a Doctor of Medicine or Osteopathy, Podiatric Medicine or an Advanced Practice Registered Nurse.
SENDING YOUR REFERRAL
When sending your referral, please include your contact information, the patient's demographic and insurance information, a home health order specifying needed home health services (SN, PT, OT, ST, MSW, HH Aide), face-to-face encounter attestation, and progress notes of the encounter, and fax to (708) 924-0501. Upon receipt and review of your referral, we may also ask for additional clinical documentation that may help with obtaining insurance prior authorization for home health services. We will coordinate with you with diligence and efficiency to ensure the patient's smooth transition to our services. 

WE ARE COVERED
We are certified by Medicare. We are also in-network with some health plans provided by Aetna/Aetna Better Health, Meridian, Molina Healthcare, WellCare and Blue Cross Blue Shield of Illinois.
